re*be® is developing an inhouse electronic medical record to assist us and you in preparing insurance forms and documents to assist your insurance company in covering your encounter with re*be®.

What to Bring

• insurance card or cards (if you have Medicare bring your supplement card too).
• Bring identification.
• If you are under 18 years old you will need a parent to give consent before you can be treated.

Even if you are coming for cosmetic reasons occasionally issues that need attention will be discussed and your insurance may cover them. Soon many of the forms we need from you will be available to fill out on line. You may download and fill out our Uniform below. Filling it out at home will save you and us some time when you arrive at re*be.

This form is the one you will be asked to fill out when you arrive at re*be. If you have a printer, you are welcome to download, print this form at home and fill it out. This will save you some time when you arrive. Don't worry if you don't know how to do this or it doesn't work on your system we have copies of the form. Our staff can assist you when you arrive too.
