re*be™ Clay County Fair Daily Drawing

how it works

You need to get and fill out an entry form with your name, address, email address and phone number
Please give us enough information so we can contact you if you win a prize.
Please let us know if we can include you in our mailing list for future re*be™ discounts and prizes by checking the box on the entry form.
To get an entry form you or someone with you must be wearing a re*be tattoo at the Fair anywhere on some exposed skin.
We will have the re*be™ tattoos at the re*be booth in the Events Center at the Clay County Fair.
You also can get the removable tattoos at the re*be™ Clinic in Okoboji also.
You can pick up a new entry form for yourself each Day at the Fair with a visible removable tattoo of course.
Your friends may also pick up an entry form and give it to you, one form each day they come by the booth with a visible tattoo.
There will be drawings at the end of each day for the prizes listed for that day.
If you win a prize your name will be recorded and your entry form returned to the jug for the next days drawings.
You may win more than one prize even if you enter the drawing only once.
Everyone over 18 years old is eligible except for the 11 current re*be employees.
To win a medical type prize you have to be medically qualified.
This will be determined by Dr. Kolegraff, Dr. Sandberg, or Dr. Ver Steeg closer to the time of the procedure or treatment.
We will reserve the right to make reasonable value substitutions for a prize if you do not want or qualify for it.
You may transfer your prize to another medically qualified recipient subject to our approval.
There is no cash value for any of these prizes.
Our intention is to offer prizes of increasing value each day of the Fair.

Prize notes:
no substitutions will be allowed for hats or robes
Latisse® is a prescription medication and a medical evaluation will be necessary to obtain your Latisse®.
Not everyone is a good candidate for Laser type hair removal procedures. You will be advised by our staff or the staff at Okoboji Skin Care if your are a good candidate.
Generally it takes several treatment to get a good result for long term hair removal.
If you want more treatments you will have to pay for them.

The drawing for the breast implant is for one silicone gel implant only.  Generally it takes two implants to do a cosmetic breast augmentation but not always.  There are other costs associated with getting a breast augmentation and these will be extra.  Examples are the cost of installation and the cost of the second implant.  Follow up care also can incur additional costs.
